1. Background
The risk cover for death and disability offered by the finpension 1e Collective Foundation is provided by Schweizerische Mobiliar Lebensversicherungs-Gesellschaft AG. In order to insure these benefits, data processing and release must be authorised. It may be necessary to answer health questions in order to grant insurance cover. The Foundation and the reinsurer will decide on acceptance into the insurance on the basis of the health questions. They may also order a medical examination.
2. Conditions
The insured person consents to the foundation and its insurer and/or reinsurer processing the data required (as name, date of birth, etc.) for the risk assessment, the assessment of any claims to benefits and the execution of this contract. If necessary, the data may be passed on, in particular to employee benefit institutions in which the insured person was or is enrolled. The foundation and its insurers are entitled to obtain relevant information on the benefit history to date from third parties, in particular medical professionals and their assistants, authorities and social security institutions, as well as employee benefit institutions which the insured person is or was affiliated, especially for the purpose of risk assessment and administration of claims. To the extent that it is necessary for the risk assessment and the assessment of a claim to benefits, this consent extends to the procurement of particularly sensitive personal data (such as health data) and personal profiles or to the perusal of official files. For this purpose, the insured person expressly releases the medical professionals and their assistants from their confidentiality obligation.
If coordination with other contracts by which the insured person is also insured with the same insurer for employee benefits is necessary for the execution of the group life insurance contract or for the processing of a claim, the insured person consents to the insurer to disclose personal data (incl. particularly sensitive data such as health data) for processing to third parties in Switzerland or abroad that are involved in the performance of the group insurance contract or of another contract by which the insured person is also insured for employee benefits with the same insurer, in particular to coinsurers and reinsurers, to employee benefit institutions in which the insured person is or was affiliated, and to companies of the insurer that are involved in the fulfilment of the insurance relationship.
